Fulltext PDFOil palm phenolics (OPP) are water-soluble bioactive compounds recovered from aqueous streams generated during oil palm fruit processing. Unlike the lipid-soluble constituents conventionally associated with palm oil, OPP contains phenolic acids and related compounds that have attracted increasing biomedical attention. Recent human trials indicate short-term tolerability and suggest effects on antioxidant status, inflammatory biomarkers, and blood pressure. However, the pathways explaining these responses remain incompletely understood. This qualitative literature review critically examines OPP through the emerging polyphenol–gut microbiome–metabolite framework. Evidence concerning OPP composition, gastrointestinal bioavailability, microbial biotransformation, intestinal barrier regulation, short-chain fatty acids, bile-acid metabolism, metabolic endotoxemia, endothelial signaling, microbial metabotypes, and precision nutrition is synthesized. Particular emphasis is placed on protocatechuic acid and p-hydroxybenzoic acid as mechanistically informative phenolic compounds. The review proposes that future OPP research should move beyond conventional antioxidant endpoints toward chemically standardized interventions combined with metagenomics, metabolomics, foodomics, vascular and inflammatory phenotyping, and responder-stratified analysis. Such an approach could clarify whether microbial transformation explains interindividual variability in OPP responses. OPP therefore represents a potentially distinctive convergence of palm-processing valorization, functional-food development, microbiome science, and preventive nutrition, although current evidence remains insufficient for therapeutic claims.
