Fulltext PDFA Forty-eight-year-old male diagnosed as metastatic nasopharyngeal carcinoma with lymph nodes and lung metastasis. In addition to the local symptoms, his presentation included high grade fever, and severe joint pain with significant leukocytosis) above 50, 000 109/L). He has no clubbing and no obvious joint abnormalities. He received 2 lines of palliative chemotherapy. His above mentioned symptoms, and laboratory findings (leukemoid reaction) suggestive of paraneoplastic syndromes), were dramatically responding to the systemic treatment. A Bone scan was done to exclude bone secondaries.
Tram- track or double stripe sign in bone scan is suggestive of hypertrophic osteoarthropathy. Its presence indicates symmetrical linear increase in Tc-99m in the diaphyseal and metaphyseal surfaces of long bones ( Figure 1)[1].
