Pancytopenia and Mucositis Due to Methotrexate Overdose in A Patient of Rheumatoid Arthritis

Abstract

Methotrexate (MTX) is an antimetabolite that was primarily developed as a chemotherapeutic agent to treat malignancies but later became used widely to treat rheumatological conditions. MTX-induced toxicity is mostly dose- and duration- dependent. Myelosuppression is a rare but severe complication of MTX that can occur even with low doses used for inflammatory conditions. The following report describes the case of a 60-year-old female presenting with complaints of erythematous rashes over the limbs and oral hemorrhagic mucositis. She is a known case of rheumatoid arthritis and was on MTX 10mg weekly but the patient had taken it inadvertently daily resulting in toxicity. She was hospitalized, MTX withdrawn, and treated with folinic acid. Early recognition of methotrexate toxicity can result in prompt initiation of goal-directed therapies, leading to better outcomes.
